Google AdSense site banned or Google AdSense account banned
If Google banned a site from using AdSense code on the site and the owner of the site is using the same AdSense code on another site, is the ban going to affect all the site that is using the code and the Google AdSense account?

Yes, once the account is banned you cannot get any benefit from those codes even if you use them on other websites.
You need to apply for new account but with different name, website, address, IP etc.

Excessive profanity
To be honest I don't know what Google considers to be excessive profanity, and I've heard different numbers from different people. The point is don't swear it looks unprofessional. Save the swearing for rant blogs or that you don't make money from.
Promoting the following
If your website or blog promotes the following your account will be banned
a. Any weapons such as knives or guns (I have seen some exceptions to this rule)
b. Porn
c. Tobacco, alcohol, and drugs
d. Promotes hate
Made for AdSense sites of MFA's
These are websites that have been designed specifically to get visitors to click on their ads. Generally they provide either very little actual content, poor content, or copied content. Either way these kinds of sites offer there visitors nothing except to try to trick you into clicking there ads.
