Search engines rank web pages on the basis of relevant content, title tag, heading tags, alt tag, meta description, keyword density, prominence and proximity.

Yes, i agree with your information. Anchor text also help you. In my opinion Location and frequency of keywords on a web page is the most important part that considered by algorithm for giving the rank.

Every Search Engines had their own criteria for ranking webpages and they place the webpages/sites in their SERP according to their own criteria.

It is really very complex algorithm to understand how a search engine ranks pages, the algorithm may be different for different search engines.
